An Italian article regarding anamnesis with a corresponding title - Quando la cartella clinica è terapeutica... with a similar content of this file/chapter - appeared in the section Spazio a chi scrive [Space for the writer] in the November 2004 issue of Magazine Torino Medica (Turin Medical Journal) as auto-presentation regarding publications and presupposed methodology. The article was however subject to space restrictions and was therefore not as exhaustive as the author would have liked. But many years before, some scripts appeared currently from 1960s in the magazine Giornale dei Genitori (Parents' journal) as column titled Childhood: a very difficult job, together with the fruit of a continuing collaboration with Montessori's Birth Center, and also with other magazines and as participation in various conferences. So and gradually some of them develop into new ones and/or become chapters of books.

Children? Parents? and their milieu? Reciprocally these topics - decidedly derived from direct CONCRETE experience and never only from simple theoretical studies - could have given enlarged and more precise way of observation and synthesis: strictly intended to evolve CIRCULARLY towards more incisive observation and consequent explanation of facts.

Important amongst these articles, conferences, communications etc. was the series of conferences held in a Round Table on the subject of Violence against the child, promoted by the Italian Women in Medicine Association (Associazione italiana Donne medico). These public series of Conferences occurred at the main office of the Order of Medicine Doctors in Turin, and then were published on papers of the same name which title I am translating in English as I am doing for every Italian title. Conferences and debates were thereafter reorganized in more organic form and their second volume transferred into a “real” book: Childhood: times of mutability, complementary with another already edited book - which first edition's title was The complexes of the housewife - now entitled Domestic chores. Ergonomy and Psychology of a REAL work 
Than to improve and broadly consider this subject, a series of publications and conferences - also International - came into being in the field and in the discipline of Psychosomatic medicine, with its related topics as for a main example domestic accidents. Part of this wide collection, selected for printing in Italian books and just in this Web site - Domestic chores. Ergonomy and Psychology of a REAL work, Childhood: times of mutability). The successive Web site - then in turn issued on the Italian printed editions of the site - Children of YESTERDAY = today's adults. Today's adults ->Adults of TOMORROW  presents and explains books, upgrading these topics on making timely use of every new attainment - either cultural or better from first hand data - and encouraging to prepare new editions of scripts and to open redesigned points of view in this field.

Childhood: a very difficult job? Individuality, development, evolution?

It is more than "banal" - rather frankly obvious - that parenthood should be considered a far-reaching and very difficult job. But not every people does be or will become parent or at least has to "manage" own or others' children. (look also on the Swiss site ASPI, and/or write to GeniAl)
Instead infancy - the impressive Latin word infans means not able to speak - and childhood are or were the absolutely unavoidable and basic steps for EVERY ONE: towards their whole existence. Therefore compared with the difficult and responsible - secoundary - parents'job, how more momentous is the primary so often more difficult - childhood's job! Therefore if often urgent is to help parents, how more  fruitful is to help every one at least to a better knowedge of this prime, crucial job!

1. Individuality: possibility and difficulty present or absent in the environment: ROOTS and the EARTH
2. Development: with its regularity and concrete firmness: the TRUNK
3. Evolution: almost infinite in its differentiation and expansion: BRANCHES, LEAVES, FLOWERS, FRUITS, SEEDS....

The tree, like every living being which “grows” mostly or only in width, must be rooted well in the ground and must be surrounded by a nutritionally and climactically favourable environment in order to reach its full completeness. However, the animal and much more the human being does not only “grow” but “develop: phenomena which follow well defined PHASES, as predisposed paths towards changes (plural!) in a biologically determined sequence, guided by a typical rhythm of physiological developments modulated both by tempo and modality, regardless of all episodic changes that every true existence goes through.
The tree is a convenient metaphor, but many other factors can be considered when referring to human existence and to every human activity: every element of human society is certainly not monolithic like a tree neither in space nor much less in time.

Adults of today -> adults of TOMORROW, at first sight the beginning and base of the aforementioned arguments could be seen as small, insignificant, extremely private if not typically feminine, despite the real fact that occupying oneself with the existential complications of  children’s development and with domestic life does involve not only insignificant and private situations as well to some degree the cornerstones of present society and future one.
Therefore and factually this not-childish matter
  1. involves a robust CONCRETENESS 
  2. argues against feeble, “airy” theoretical hypotheses;
  3. sets against inconsistent, ephemeral fashions and dictates;
  4. warns against episodic descriptions: not meaningful but silly over-sentimental, empty as generalizations, and then capable only of impeding any recognition of the concrete connections between cause and effect
  5. protects against ways of thinking unrealistic and dangerous: themselves cause of a direct “violence” (this was the basic argument expounded in the conferences on Violence against the child);
  6. defies and advances against the scorn and the repudiation of MEMORY;
  7. maintains finally IN FAVOR of completeness of FOUR dimensional beings: of TIME and not only in a poor and static three dimensional concept of space.
In fact, if a mature human being has to become so much powerful and versatile each stage of this development should leave behind - like a lesson well assimilated - a solid imprint. This integral fulfilment asks that each step of his/her completing could receive an acknowledgement of the typical, concrete and precise psychological and/or environmental NEEDS (n.b. needs not “desires”), an acknowledgement really active during each precise phase of his/her journey. 
Harmonious in its typical discontinuity each complex sequence manifests itself as physical changes in the body and, in concurrence with this, the prevalence of certain functions and relations, affection and feelings, activities and preferences... Also - phase by phase - this occurs to more easily activate a fitting constellation of instinct: a very mighty extra-personal power arousing at times to connect individuals with environmental elements. Each one of these groups of coordinating factors scientifically is named constellation: but compared with a musical complex it is worthwile not to change the by now classic term complex given by Freud more than a Century ago.
In humans in particular this preparation is lengthy and arduous. To a so extremely complicated network of connections between past and present is intrinsic a peculiar fragility (why then this title: Childhood a very difficult job?), since that "complex job" is so much more difficult than that scholastic preparation of material/learning, both not attainable without adequate contribution from outside.
A brief essential semantic note is to be recommended for the terms development and evolution, a suggestion so much required to be shared not only in this file/chapter but also - exactly quoted - in other ones. Evolution means something more broad in term on space and time than development: only entities, already present and still "involved" as buds, can develop, while after every moment the ambiance's stimuli can modify and also enhance every life.
If every moment living beings have to act for maintain the internal homeostasis, nevertheless "well built", "well developed" sturdy organisms and minds can not only "accept" but also make the best on integrating themselves with the new situations: they can not only "learn by experience" since even transform every occurrence in  great creative forces which can promote - one could almost say GENERATE - evolution in new and totally unexpected directions. But going again back to the simple regular to sprout: how should "uncoil" its passages in a favorable coordination - preordained even if instead so "embroiled" in their coming next each other apparently discordant - without demanding, external variant, if not "acrobatic", assistance? 
Precise and mutable support systems are then needed for each phase, but they should themselves be flexible in responding to apparent irregularity, and concentrate carefully on the precise galaxy of emotion undergone at that moment. Already from its title, Childhood: times of mutability, describes and clarifies the physiological - carried out or disturbed - changes in expansion and  differentiation from the point of view of the developing subject, whilst Domestic chores. Ergonomy and Psychology of a REAL work considers the adult protagonist, contributor and participator in these developmental stages. However to be "adult" means  to have been child: not only assistance, home work and children rearing are directly concerned with the development of the successive generation, but as well with the good or bad possibilities to have undergone personal past phase, or rather with the "order or disorder" of the preceding generations.

From children of yesterday to adults of tomorrow...?

The process of development is so varied in its multiple integrations and depends on such a precarious balancing of body's physiology and completeness that it can easily be castrated, moreover if "polluted" by an alike previously mutilated environment. In connecting to rearing children obliged times-linked relationships - so changeable and overloaded with emotion, or better merging on the main field of generative instincts - each one of us is anyhow an unavoidable determiner of future scenarios: unpredictable and collective. The gradual development, the accumulation of each preparatory stage and the strengthening of the ground for its successor can, however, easily become mutilated in direct or indirect ways, leading to real emotional castration of the single individual or of entire populations.
Childhood: a very difficult job?
The galaxy of emotions which mark phase by phase of development are particularly varied and often shifting in the orbital of apparent contraries. These powerful yet fragile elements and the extremely lengthy preparatory stages, seemingly discordant - yet well coordinated by nature - are exactly those which serve to integrate gradually the power of the individual with discordant environmental factors which can be simplified on three orbits following each other towards superior steps: either personal, or regarding deeds, rules, facts, ideas, "things", or else people's relations and integration, "sentiments" and dealing. This difficult balance requires at the same moment a precise, powerful and well established external support system which centres on these emotional states, activated moment by moment and, above all, phase by phase. To an inattentive or forestalled observer these necessary delicate passages may escape detection and it in a way that unfortunately too often happens: in this way which could seem insignificant if not subliminal, crucial moments are ignored or blocked, leaving further cascading consequence. Thus extremely grave future consequences tangled up in - absurd - inextricable complications, can be induced almost inadvertently, almost unconsciously not by colossal errors, not by cynical ill-will but merely by the lack of swift help given to ignored phases of development and by lack of management of unaccepted emotions.
Unrecognised and/or unfulfilled precise contingent needs tend, then, to leave in their aftermath a precise, determined emotional incompleteness and/or deviation, not only in the present but with the above mentioned cascading effect on the further whole of individual’s existence, producing monstrosity - analogous to the disfunction of the organizer-inducer of the anatomic development in embryo described by Spemann and Mangold.
All these issues emphasize an united discourse insistently proposed, highlighting concepts of beginnings, of successive passages, of phases of development, of past, present, future,  organizers and cones of expansion which integrate and interact. Also insistently underlined however is how, when these phases meet obstacles, become vicious circles, which spin around themselves also blocking successive phases, if not continuing to aggravate towards pathological directions, branching off from the single individual and overcoming - in the present, but often well in to the future - entire existences: the affected subject and many around him.
As already outlined above, in every instant LIFE - in capital letters and merely as itself as the contrary of "dead" being - must be underlined by the particular chemical-physical continual changes in its internal state, leaded also by emotions as an important foreground of general reality of living being. "Emotions" are - by actual word definition - variable and also numerous; usually they follow one after the other in contrasting manner in the same individual body: in a matter of minutes this kaleidoscope of states alternate and often oppose “mood”, “attention” and also every psychosomatic form in interaction into individuals themselves and between individuals and their complete environment. Any way all this does not frame an ambivalent simultaneous ambiguity than a "tumult" into which emotional states separate each other by short, but not null delaying interval, initially uncoiled but prepared to be solved and recognized. Then instead of ambiguous uncertainty it should have a powerful function: it should just heighten tensions, which as elevated potential differences change into available hightened real voltage accumulators of energy.
But not ever it is feasible to engender more power, rather and on the contrary when discordant emotions are experienced as subjectively insoluble contradictions, as well when possible high-tension improvement muddles in an inextricable coil, then this differences of voltage can't be beard - even till stirring real seizures. Internal, unbearable conflicting “emotional experiences” moreover lasting from childhood, as at any age to try to artificially improve a not ready body for exceptional performances very often not only act against useful differences on voltage, but instead this to increase the potential energy explode in manifestation of possibly serious “psychic” symptoms, like anxiety or panic attacks or even fainting, and also in real dysfunction if not harm - till "broken heard death" - to internal organs: arteries, heart, digestive, endocrine systems, etc.
Not only emotions exist however: the human being is also aided in his interaction with his environment by way of INSTINCT, a powerful biological chemical-endocrine faculty (and not only), triggered by stimuli received by way of immediate sensorial reaction, a faculty which, compared to the “emotions” can affect a much larger scale of space and time and provoke much more intense reactions. The instincts are an immensely powerful system for the protection of individual existence and/or the species as they effect the action of coordinated and lasting hormones and not only the more ephemeral neuromediators and neuromodulators. The endocrine system is complicated and delicate and the unloading of energy - electromagnetic etc - once activated modifies every surrounding factor. If the instincts in action can cause enormous changes in one’s way of being and situate oneself dynamically in reality, they can either proceed in an appropriate way or in a chaotic, destructive way - see, for example how the same instinct either “saves” or “panics”, “to die from a heart attack” and certain miraculous survivals.)
In sum up, every living being’s emotions and instincts construct around himself what generically one can call “atmosphere”: in every moment - both in adults and children - this is a transforming factor, which either disturbs or acts as a link in the vast and beneficial chain of communication. 
And then: aroused and continually modified either by endogenic conditions or exogenic stimuli, the emotions follow on from each other and intersect not in hypothetical ways but by expressing themselves through their precise chemical and energetic bases: also these, like their name implies, in continuous modulated movement.
To specify again: every existence continually develops many ways to interact with the immediate present and in varied and continuous correlation with the environment, as already mentioned above recognisable even by their chemical structure as reliable “scientific” evidence of internal homeostasis. But not only this: alongside the chemical-physical visceral system of dynamic adjustment to the environment lie other relational systems - movements and sounds - which integrate every living presence. In particular in human beings exist words, exists to speak a special and sophisticated instrument for the transmission of information: even not only for "external" trasmission..
A fruitful fusion of mind and body, which continually transforms the capacity to sense emotions and feelings, can then turn everything into true EXPERIENCE: the only way to generate timely fitting personal CHOICES. But every real BEING isn't an isolated entity: a wrapping unabridged environment continually is moving around and interfering with. The common place description of Instinct is almost ever belittling these extra personal forces considered instead only from a restrict behavioral point of view. On the contrary INSTINCTS as mighty powers support - in not castrated organisms - the vital possibility to handle very timely also special happenings; instincts have the MAIN DYNAMIC TASK to connect immediately straight away the individual, its feelings and actions with the environmental stimuli.
Instinct is the inherent disposition of a living organism toward a particular behavior ... Instinct provides a response to external stimuli, which moves an organism to action....
Any way if Consciousness and memory file/chapter is so strictly related to this one - to have even some quotes shared together, besides just on regard of these problems - Sublimations: visual schemes and scientific explanation outspreadly defines from a different point of view and with more meticolous exposure the modes, ways and patterns through which every living being interacts with and into the sourronding milieu.

Other factors exist to complicate matters, atmospheres and relationships such as present contingencies and life in all its versatility and wealth of continuous variation and episodic change
In each living being chemical and energetic expressions of biological, powerful, too quick to become conscious INSTINCTS and EMOTIONS manifest themselves not only in gut reactions and experiential modifications but in objective factors as well: then surely not to be confused - as often does common place people - with the simple so called intuition, only a quick and ready insight, a somewhat more speedy as usual possibility to conclude thoughts and to recall deeds to mind. The main powers given by instincts and the antennae assured by emotions are not only behavioral, not only psychosomatic but also “somato-relational”. It is therefore inevitable, in circular and intersecting connection that each body receives and exchanges, that each body exudes emissions into the surrounding environment; uncontrollable emissions transmitting volatile chemicals called pheromones as well as other variants in magnetic field etc. transforming each simple living presence into an important factor of effective causality: thus perhaps the more important issue regards not the emission of pheromones but the receptivity and responsiveness in the receiving organisms.
It is then to point out that emotional doesn't mean “inconsistent”, a state to undervalue or ridicule: the emotions and their derivatives and effects are not adjectives meaning simple definitions, but are, in their concrete substantiality true nouns, which indicate the emergence and variation of ar least concrete chemical molecules not hidden inside the organism but spread around. These chemical expressions of internal states have often a short duration, a not so currently definite smell and so then often are difficult to be considered: notwithstanding - noticed or disregarded: why even people can use to smell as synonym of to foresee? - they are not only typically vital manifestations of the visceral-somatic sphere but their activation colors and gives meaning to every moment of global existence, also if as a rule far from perceptive consciousness. In an individual these internal autogenous transformations and triggered environmental modifications, gained by means of a well refined sensitivity - always been present or coming from a recovered awareness - can, or better should, transform every one of life's episodes into lasting EXPERIENCE and permit a whole consciousness to expand till accumulate into wisdom. Any way at the same time chemical emission of volatile embodiment of instincts and of emotions as well as every physical change determined by other variants in magnetic field etc into the surrounding environment transforms involuntary every simple living presence into an important source of change which should be taken into account.
Thus every living organism is at every moment - too often without being aware of it - a greater or lesser disturbing factor to those around him. On the other hand the organism can become part of a vast communicative chain mentioned previously: with his or her emotions each one contributes towards the building and redistribution of that which we generically call atmosphere; that diffuse atmosphere that is the base of the oral complex: that “oral complex” which can be summarily exemplified by those words which refer to “entrances”, “acquisitions”, completeness and also the whole series of words, verbs and nouns in which are presented concepts of togetherness, of diffusion, and with prefixes such as “con”… “syn…”, or with terms containing the idea of “fusion” , such as the previously given “diffusion”.

Nosy Neurones: a tale of sex and the sense of smell In our daily lives we are surrounded by evidence of the powerful link between our sexuality and the sense of smell. Think only of the multi-billion pound perfume and deodorant industries! It appears that our reproductive processes are manipulated, not only by these expensive artificial odours, but by nature's own airbourne chemical signals, known as pheromones. In a recent article in Nature, scientists have shown that human pheromones can alter the timing of ovulation in women. But what is the basis of the association between our fertility and the sense of smell. For an explanation we must look at the development of the brain centres that regulate these two functions

Pheromones in Human Gynaecology
This essay proposes only to draw attention to the question of pheromones in the human sphere; an argument full of questions and and subject to varied explanation and expansion. (Pheromones are those hormonal substances which chief characteristic is not to act “within” the organism secreting them but on relational behavior and on some physiological balances in other organisms, reaching them in infinitesimal doses and moreover in a volatile state.)
The ethological study of animals has developed in two different directions; that of aggression and of sexual function, above all female, including that which runs in conjunction with the latter, the arousal of the male. Few studies have explored other fields e.g. “learning”.
Regarding female sexuality the case-study of female rats bred in complete absence of male rats is particularly demonstrative; only minute traces of masculine urine is necessary to recover a normal oestrus, fertility and sexual “purpose” - meaning heterosexual - in rats otherwise “condemned” to a complete homosexuality and the lack of an oestrus cycle. Or, still in female rats, the spontaneous expulsion of fetuses and the reappearance of oestrus in pregnancy set off by the presence of a more “preferred new fertilizer" male. Few studies on the human field have been published up to now, the most noted being those regarding female sexuality: the synchronization of menstrual cycles in women living together, for example students in American colleges who share rooms.
Personal observations conducted over some (five) years can be added to this well observed phenomenon of human menstrual synchronization (corroborated by the synchronization of oestrus in female animals). I refer to the synchronization of menstrual cycles in myself and my daughter over a period in which we passed a certain number (average three-four) hours together each day collaborating together in a communal activity (I have the impression that this was a determining factor); and vice-versa to their de-synchronization when our occupations diverged. Also, in the animal sphere, the synchronization of oestrus in my two female dogs, despite variations in the frequency of oestrus (intervals of between six to eight months). In this case the interval, constant and regular - of six to eight days between the start of oestrus in the older dog and that of the younger - gives weight to the hypothesis that the “induction” in the younger was the work of “pheromones” emitted by the older dog. 
I do not present in this document an experimental work and therefore do not wish to expatiate with tables documenting the chronology of these pheromones, but for using this opportunity to propose various questions and hypotheses.
Sexuality, aggression, learning etc: it is probable that these substances have vast repercussions on all aspects of physiological behavior and relationships in the biological organism and within social gatherings. But why do we know so little about them? What inhibition or lack of curiosity has impeded us from bettering our knowledge through technical verification? Could it perhaps be due to the fact that their effect is felt directly in the centers of the brain which are home to a protopatic sensitivity, and being triggers of vegetative reflexes, we are not stimulated to take them into epicritic (and therefore “critical-rational”) consideration as we do with visual and auditory sensations? Or - mostly - are we not yet culturally permitted to take them into consideration, because if we admit the presence of these ways of communication, of involuntary interpersonal messages, we must also accept the impossibility of intentionally falsifying them? (Won't this be the reason for the real phobia against body odor and the huge market in deodorants?)
Addressing the female genital apparatus and its functions: could not the functions of the peri-vaginal glands (as well as all the apocrine glands of the body) be revalued in the light of this?
Could not the inhibition - maybe for psycho-relational motives - against the expulsion of secretion, having a pheromonic effect be amongst the causes of bartolinite? Furthermore, as the menstrual or oestral cycle can be “directed” by another’s female cycle, or oestrus induced by the odour of masculine urine, could not human fertility, in its various stages, be conditioned by the influence of pheromones in the environment, male or female?
To return to general problems: what could be the influence of substances - alimentary, voluptuary, medicinal, contraceptive etc - introduced into the body on the conditioning of other organisms via thus modified pheromones? What is the effect of cosmetics, detergents, localized contraceptives, pollution etc. even as artificial pheromones or disturbing agents?
To cite an interesting example: a normal insecticide spray with a pyrethrum base, though with a propellant and perfume not specifically named, sprayed on my female dog provoked erotic attraction and excitement in all the male dogs encountered, as if the bitch was on oestrus. I am sorry not to have been able to carry out a further experiment to see if the pheromone effect resulted from that one spray can and not from others of the same brand and type. Another episode concerns instead newborn babies about three days or so: all these babies in a hospital nursery burst into tears in unison at the moment when the floors began to be waxed for the Easter holiday cleaning.
And finally: perhaps the more important issue regards not the emission of pheromones but the receptivity and responsiveness in the receiving organism: therefore what influence can the endocrine state, the emotional state, stress, pharmaceuticals and socio-affective maturity or immaturity have in this sense?

Anthropology of the individual - term proposed for a new field, makeing an interdisciplinary approach to the human individual, in his whole somatic, psychic and spiritual complexity. We have built a nuber of models for the human being: the systemic model (Fig.1), the interface modelling (Fig.2) and (Fig.3), the integronic model (Fig.4: look below). 
Body aura - universal and fundamental form of existence, which accompanies the material existence and the fields existence (gravitational and electromagnetic field). We have approached the scientific study using the electrographic method of the living and non-living bodies, and we have established a semiology for the electromagnetic signals appearing in the bodies' auras. 
Information of the aura - fundamental property, characterizing the bodies aura - present everwhere -, similar to the bodies mass which characterizes the quality of substance, and similar to the energy, which characterizes the quality of field. The aura's information shows up as archetypal information. We have highlighted the following archetypal forms: globular, ramnified, radial and floral
Integronics - this is a name proposed in 1977 for a new theoretical field which explains our experimental results, and studies the general laws of the coexistence of the three fundamental forms: the substance, the field and the aura.

To call emotions by their authentic name...

The real meaning of an instrument such as words lies - or perhaps only should lie (?!?) -  in the transformation of things, facts, situations of any type - including the selfsame emotions - from concrete to abstract, from imprecise, “plodding” concreteness to ethereal and effortless ways of expression and of reciprocity, not only easily interchangeable but also, in actual fact, capable of being precisely defined.
For all these reasons practical and/or scientific medicine cannot disregard or set aside the necessity for a very accurate anamnestic understanding of the present and the past (however remote) history of the patient in question. Here we further explore the title Anamnesis? A way for healing... and link to the sources and the aims of the books presented in the site and of the site itself with its dynamic refreshing in updating progress. 
The aim of words, even if often disregarded is - or should be(!) - that of giving a name to everything. Starting paradoxically from this new abstract basis, to give proper "names" can restore a renewed, precise concreteness: this is the great therapeutic gift offered by case history, by the simple taking of anamnesis: since a "banal" collecting of information, linking all to exact names, can gradually make possible the expression of - or even give rebirth to - complete relived experiences.

If accepted during an impartial hearing, interior occurrences instead to remain inside to rot can, gradually, come out from the depths of feeling and memory, finding with their true NAME their exact place in existence and in the wealth of lived experience. But development has an obliged natural own intrinsic regularity, a basic regularity which can be found in another enormous gift: the past gathered to live into MEMORY, into this well organized "hyperlink gallery" of gathered life stories but also in sum up where to reach a prepared deposit also for only presumed, not yet realized life agenda necessary occurrences. At this point intensive and not only psychological changes can take place: often so rapid and intense to seem…. “magical”. The loosening of accumulated tensions in deep-rooted thoughts without voice is - however certainly not “magic”! - when finally the “recovered voice” is transformed into a recognition and a restitution of the true and proper identity and of the true and proper - present and past - “presence in the world!” 
If this instrument of clarification and of condivision is undervalued it would take away the basic efficacy of Medicine as a practical profession and the scientific needs of medical research, but blind faith in this method without cautious attention could give illusory and deceptive results. Such enormous practical implement could in fact be in vain if the speaker is incapable to use it appropriately or from defects in translation in the listener. Words are used for pointing out "deeds" but not ever these deeds are the same for the interlocutors: every one believes to be understood or to understand when instead from this deafs' speech rises a series of misjudgements and fake promises. Besides: since words are instruments controlled by “will”, it is possible to tame them either by the speaking, maybe lying person, or by the almost on purpose deaf listening interlocutor: it is to say that language not only should be a valid instrument to unveil, uncoil, helping to develop, but alike it may be as much badly managed, badly interpreted if not also - on purpose - used for misrepresentation or deception [a synonym of involved isn't imbroglio/misleading meaning also TO DECEIVE/FRAUD?]

Most recent advances stem from groundbreaking research first published in 1973, when scientists discovered the brain's opiate receptor and established the basic technique for studying brain receptors still used today. The opiate receptor and other brain receptors are proteins located on the surfaces of nerve cells, or neurons. The brain works through neurons communicating with each other by releasing signaling chemicals called neurotransmitters. These chemicals attach to receptors on nearby neuronsthe way a key fits a lock. Prior to 1973, scientists understood little about opiate drugs or about drug action in general. In fact, how can a careful and critical report of the emotional and lifestyle state at every encounter between doctor and patient not be taken into account? How can we admit a scientific doctrine which presents biochemical processes as immobile when there are present typically dissipative phenomena? Chemistry of the emotions....: how could be considered admissible as “scientific” a "doctrine" insisting on pseudo-therapeutic proposal which presents such intricate, tangled processes as simple and static
In technical terms the study of causes is called aetiology, while the study of their mechanisms of action is called pathogenesis; both these can be called true Science only when they are well verified and not hyper-simplified; when they are truly considered in their essence as well as in the dynamism of their evolution. It is only then that an abstract idea of pure Science passes towards eventual verification, repetition and calculation, comparable observation and, if it is the case, also experimentation.
One of the basic branches of theoretical medicine, together with doctrine and research is called, in every language, General Pathology. This science deals with all the varied and composite systems of cause, concomitant cause, situation and interaction. In sequence with this and with the study of normal Physiology lies the study of the dynamic homeostasis of living bodies in their well being and well managing till even evolution, as rather in their disharmonies. Rarely do isolated causes account to modify the normal performance of vital phenomena, in the normal interaction between these and those: instead much more frequent are the groupings - the galaxy constellations - of primary cause and concomitant ones and their varied and interrelated possibilities of action. Neither can a hypothetical state of health be considered as a certainty: health is not a static concept but is a moment hung in the balance, the delicate equilibrium of which defines the life of the subject.
A true physiology of every effectual existence cannot disregard together with the multiple correlation of the present also the various modalities enacted to perform it. Emotions, relationships, feelings, affection and subjective experiences forge part of the actual reality of the whole body condition also of its reality defined as psychological. Undervaluing all this undermines the basic efficacy of medicine as a practical profession and damages the scientific basis to recognize the true plurality of authentic logical links of causality. But not only the ephemeral moment has to be noticed: also a doctor himself however - as well every one who cares for or even only surrounds every person till from his/her first approaches towards life - is a real "person" with his/her own nature,  revealing himself as coming from his own past: also for and into him/her is ever acting out a good or bad past, well or badly experienced, well or badly integrated in the real "to-be" of the present moment. Stated again in more explicit terms: it is not enough to say that, if every data is not correlated with the psychological part and moreover PAST existence of the patient's examination and  with the knowledge coming from doctor's true experience, that every diagnosis which doesn't also contain the maximum of this type of information it cannot but give defective results.

The inability to emotionally regulate can manifest in the symptoms of depression, anxiety, insomnia, somatic illnesses, loneliness and conflict that our clients bring to us. That same emotional arousal feeds our runaway addiction to substances, food, sex, work, rage, electronic devices and the compulsion for constant activity. ... The importance of the right hemispheric healing goes on beneath the words and the particular theoretical clinical approach. [To] briefly explore our work with our client’s relationships, (couple’s and family therapy) building an understanding ... requires that we, as therapists, do more than teach “good communication skills”. [However ...] the knowledge of how “to be” in intimate relationship [gives] ... an empowering explanation of how we act in our offices, as “neural architects”, reshaping and remolding our client’s brains, and engaging so deeply that the work profoundly reshapes our brains as well.

How therefore can we not take into account carefully and critically the reciprocal use of language and not only the concomitant emotional state in every encounter between doctor and patient? How can we not risk misrepresenting the case picture undervaluing fundamental elements and/or overstating others, misleading appeals confusing with physical symptoms, or on the contrary paying attention to real deceptions passed off as requests? How can colossal errors be avoided, instead to confirm and confirm them again by themselves in purely numerical successive statistics? And if mathematics is at the base of every serious application of science, why then subordinate the mathematics of shifting algorithms to those of fixed statistics, so often full of badly chosen numbers?
Contradicted and Initially Stronger Effects in Highly Cited Clinical Research

Context Controversy and uncertainty ensue when the results of clinical research on the effectiveness of interventions are subsequently contradicted. Controversies are most prominent when high-impact research is involved. Objectives To understand how frequently highly cited studies are contradicted or find effects that are stronger than in other similar studies and to discern whether specific characteristics are associated with such refutation over time. Design All original clinical research studies published in 3 major general clinical journals or high-impact-factor specialty journals in 1990-2003 and cited more than 1000 times in the literature were examined. Main Outcome Measure The results of highly cited articles were compared against subsequent studies of comparable or larger sample size and similar or better controlled designs. The same analysis was also performed comparatively for matched studies that were not so highly cited. Results  Of 49 highly cited original clinical research studies, 45 claimed that the intervention was effective. Of these, 7 (16%) were contradicted by subsequent studies, 7 others (16%) had found effects that were stronger than those of subsequent studies, 20 (44%) were replicated, and 11 (24%) remained largely unchallenged. Five of 6 highly-cited nonrandomized studies had been contradicted or had found stronger effects vs 9 of 39 randomized controlled trials (P = .008). Among randomized trials, studies with contradicted or stronger effects were smaller (P = .009) than replicated or unchallenged studies although there was no statistically significant difference in their early or overall citation impact. Matched control studies did not have a significantly different share of refuted results than highly cited studies, but they included more studies with "negative" results. Conclusions Contradiction and initially stronger effects are not unusual in highly cited research of clinical interventions and their outcomes. The extent to which high citations may provoke contradictions and vice versa needs more study. Controversies are most common with highly cited nonrandomized studies, but even the most highly cited randomized trials may be challenged and refuted over time, especially small ones.
